Sadly because of the format of the table I cannot directly copy the Risk Assessment form onto here..
So here is a photo off it instead

To read the Risk Assessment simply click on the photo so that it enlarges, when this has happened press the control button and then the plus button on your computers keyboard, repeat this until the table is the right size for you to read, to put your screen back to its original settings press control and the minus symbol however many times you wish

so the symbols will look like this:
